How to Revoke Pro-card Privileges

Summary: The Supervisor or FAST/AP or Purchasing completes the following steps at any point during the purchase/review/ reconciliation process when intentional misuse of the card is detected:

  • A pattern of misuse has occurred -- 3 missing receipts, 3 late statements, 3 policy misuses
  • Program abuse has occurred -- e.g. personal purchases or cash refund for returned goods.
What the Supervisor does: How to do it:
1 Notify the pro-card program administrator. Call 459-3343 or e-mail the Pro-card Administrator at procard@ucsc.edu
2 Request change by completing the UCSC Pro-card Acccount Modification or Closure Form.
  1. Mark the Close an Existing Pro-card Account and Terminate Account boxes and enter the Cardholder name and account number.
  2. Sign the form and forward it to the Program Administrator via fax or mail.

Changes will be made immediately upon receipt of information.

3 Notify the Cardholder and their Supervisor.
  1. Complete a Notice of Revocation that describes the reasons for revoking the card.
  2. Deliver a copy to the Cardholder and a copy to the Pro-card Administrator.
  3. Attach a copy of the notice to the monthly statement.
What the pro-card administrator does: How to do it:
1 Complete the process.
  1. Notify US Bank to close the account.
  2. Retain originals in Purchasing (Cardholder Account Form and Notice of Revocation.)
